Claim Form - Rural Development Programme 2014-2020	The European Agricultural Fund for Rural Development (EAFRD) finances rural development programmes. Beneficiaries of EAFRD grants can claim payment from the Agriculture and Rural Payments Agency in accordance with the Council Regulation (EC) No 1698/2005 solely where expenditure is jointly managed.
Consent by Heir of Deceased Beneficiary to Transfer Pending Financial Assistance	When a beneficiary dies, it must be determined who should take over the deceased person's estate. A notarial declaration is required to provide information on the person who should take over the deceased person's pending application for financial assistance under the EU Common Agricultural Policy.
Debt Acceptance Form for Debts issued by the Agriculture and Rural Payments Agency (ARPA)	An applicant can choose to accept the penalties issued by the Agriculture and Rural Payments Agency (ARPA) after the explanation given.
Declaration of Interest for Research in the Land Archives Farmer's Registry	The application is mandatory to form the type of information needed, along with the mandatory ownership documents which must be submitted with the application form to substantiate the request.
EU Financial Assistance Bank Payment (For Local Payments)	Anyone eligible to receive EU financial assistance under the Common Agricultural Policy will receive relative payments directly in his/her bank account. No other means of payment are allowed. Therefore, when one submits an application for such EU funds, s/he must also provide bank details.
Export Licence or Advance Fixing Certificate - AGREX (AGRicultural EXports)	A licence that allows a Malta-based exporter to export agricultural produce to non-EU countries.
Farm Advisory Service (FAS) Registration	The objective of the Farm Advisory Service (FAS) is to assist farmers and investor breeders by providing professional advice. If the entity wants to be registered as a Farm Advisory Service, the Farm Advisory Service Registration Board will assess applications for recognition and inclusion of the list of entities which meet the requirements.
Farm Advisory Service (FAS) Status Report	Any Farm Advisory Service (FAS) must compile a status report every 6 months and submit it by not later than 1 December and 1 June of each year
Farmers' Register Access	Personalised web-based access to the Land Parcel Identification System (IACS).

Force Majeure Cases Request	Force Majeure relates to unforseeable circumstances that prevent someone from fulfilling a control or obligation, such as health, death of trees, livestock, land taken by owner or for public use. In such cases, farmers may request an exemption from certain obligations.
Import Licence AGRIM (AGRicultural IMports)	A licence that allows a Malta-based importer to import agricultural produce for free circulation into the European Union.
Information sent via SMS by the Agriculture and Rural Payments Agency (ARPA)	The SMS notification is an alert service sent to the beneficiary as instructed by the Agriculture & Rural Payments Agency (ARPA). It will indicate the amounts that should have been deposited in the beneficiary’s account and is currently being used to inform registrants about various obligatory courses and any agriculture related material.
Land Contestation Form by Contestant	Applicant requesting to register land parcels found to be already registered on third parties in the Land Parcel Identification System (LPIS) must file a contestation.
Land Contestation Form by Registered Farmer	This form is used by the registered farmer when a contestation is presented by a client.
Land Counter Contestation Form by Registered Farmer	Once a valid Contestation Form submitted by a contestant is received, the registered farmer shall be informed by the Agriculture and Rural Payment Agency, in order to submit a counter contestation of the land parcel subject to contestation.
Notification for Palm Movement from EU Countries (For the General Public)	This form is to be used by persons/entities importing any plant/plant material from the EU, and who are required to complete this form as a means of notifying the Directorate of the arrival of plant material at least 48 hours prior to the entry of the consignment in Malta.
Notification of Activity for Operators dealing with Organic Products	This form is to be submitted by any operator who wants to produce, prepare, store, import or retail in any way organic products, prior to placing organic products on the market.
Notification of Entry of Plants and Plant Products from Third Countries as per SL 433.03	This form is to be used by persons/entities registered with the Plant Protection Directorate and the general public importing any plant/plant material from third countries, which are required to complete this form as a means of notifying the Directorate of the arrival of plant material at least 48 hours prior to the entry of the consignment in Malta.
Objections to Reductions or Penalties imposed by the Agriculture and Rural Payments Agency (ARPA)	An applicant may object to the penalties or payments issued by the Agriculture and Rural Payments Agency (ARPA) in relation to direct and area aid schemes.
Pass Change for Free Transport of Agricultural Products from Gozo to Malta	This application is used when an individual requires to change/modify the vehicle details which is eligible for free ferry tickets for the transportation of agriculture products from Gozo to Malta.
Pass for Free Transport of Agricultural Products from Gozo to Malta	This application is used when farmers require a permit to be eligible for free ferry ticket for the transportation of agriculture products from Gozo to Malta.
Recognition as a Producer Group or Producer Organisation for Fruits and Vegetables	Council Regulation 1308/2013 and Commission Regulation 891/2007 and 892/2017 lay down the provisions for recognition and governance of Producer Groups and Producer Organisations. The scope is to establish an organisation recognised under the EU legsilative framework for farmers to team up and pool in resources and capacities for better econmies and environmental
Recognition as a Producer Group, or Producer Organisation for Other Products	The application is mandatory for new Producer Organisations or Producer Groups in order to supply their details to the Agriculture Directorate, and submit it with the respective operational programme.
Recognition for Technical Advisor for the Provisions of Fertiliser Plans	In accordance with SL 549.66 Regulation 4(i) (c), a fertiliser plan shall be compiled by a qualified technical advisor as may be approved by the Director, who will also retain a public register of such persons which shall be published on an annual basis.
Registration of Distributors, Retailers and Resellers of Nitrate Based Fertilisers	Article 14 (1) of SL 549.66 Nitrates Action Programme Regulations states that all importers, distributors, and resellers of fertilisers shall be registered with the Department.
Request for Palm Movement by Public/Private Entities or the General Public	In accordance with SL 433.24, Control of the Red Palm Weevil Regulations, any person or entity, whether public or private, is required to obtain written authorisation from the Plant Protection Directorate in order to import or move planted palm trees from one location to another. The procedure will normally involve an inspection by a Plant Protection Officer prior to granting of permission for the movement of currently planted palm trees, to ensure freedom of infection by the Red Palm Weevil (Rynchophorus Ferrugineus).

Request for the Inclusion of a Product in the Register of Traditional Agro-Food Products of Malta	Through this form, any individual can submit a request for a product to be included in the national register for traditonal agro-food products of Malta, which register shall include information of the product, its processing, preservation and the product’s history and tradition.
Request for the Provision of Courses held by the Agriculture Directorate	This application form serves as a request to register the farmer to any interested courses held by the Agriculture Directorate.
Sale of Fruit Trees	Farmers, gardeners and field enthusiasts can purchase local and imported fruit tree species, as well as rootstocks from the centre. The local fruit trees are propagated at the centre, while the imported ones are brought from the Fruit tree nursery in Malta.
Ceasing of On-Farm Mixing Activity for a Type of Feed	The form provides for farmers who perform on-farm mixing of feed to notify the Feedingstuffs and Animal Nutrition Office of having ceased to perform such activity in full or part thereof.
Derogation from Antibody Titration Test	A declaration that the owner or another authorised person will remain within secure means of transport during transit, or within the perimeter of the airport with the pet, following that there is no contact with animals of species susceptible to rabies.
Feed Business Registration Form	The form provides for whoever is interested in starting any animal feed related activity, or is already performing such an activity, to ask to be registered as a feed business with the Feedingstuffs and Animal Nutrition Office. For whoever is already performing such an activity, this form is used to regularize their position.
On-Farm Feed Mixer Registration Form	The form provides for farmers who want to start mixing, or are already mixing feed on their farm for use on their own holding, to ask to register this activity with the Feedingstuffs and Animal Nutrition Office. For whoever is already performing such an activity, this form is used to regularize their position.
