Personal Declaration of Good Repute and Good Conduct for Carriage of Passengers Operator Licence (CPOL)
Transport Malta requires a Police conduct in the following circumstances: i) When applicants apply for an Operator's Licence. ii) For Operator Licence verification (after 5 years). iii) Changes in Manager/Director/Legal representative of the operator.
Personal Declaration Form - Commercial Tombola
Applicants who would like to obtain any of the Commercial Bingo licence, as well as any associated persons, are required to fill in this form with the respective licence form.
Personal Declaration Form - Commercial Tombola (Key Official)
A Licensee cannot carry out the activities in respect of which the licence is issued unless and until he appoints a Key Official residing in Malta or in an EU/EEA State or other approved jurisdiction. Applicants wanting to enrol as Key Officials of a Licensee are required to fill in this form with the respective licence form.
Personal Declaration Form - Key Official (Remote Gaming)
This form must be submitted for every person nominated by the Operator/Licensee that shall be responsible for carrying out Key Official duties, as per Remote Gaming Regulations (S.L. 438.04). The Gaming Authority (GA) considers that Part IV of the Regulations issued under the Lotteries and Other Games Act (Cap. 438 of the Laws of Malta) empowers GA to request an Operator/Licensee to appoint at least one Key Official for approval by the GA and as a condition for a remote gaming licence duly issued in accordance to the Law.
Personal Declaration Form - National Lottery
This form must be submitted for every natural person that is a director, shareholder, CEO, Ultimate Beneficial Owner and persons in a position of responsibility of the Operator/Licensee as per applicable laws and regulations and/or for any other person that the Authority may request.
Personal Declaration Form - National Lottery - Key Official
A Licensee cannot carry out the activities in respect of which the licence is issued unless and until he appoints a Key Official residing in Malta or in an EU/EEA State or other approved jurisdiction. Applicants wanting to enrol as Key Officials of the National Lottery Operator are required to fill in this form with the respective licence form.
Personal Declaration Form National Lottery - Money Laundering Reporting Officer (MLRO)
This Declaration Form must be submitted for every person nominated by the Operator/Licensee that shall be responsible for carrying out money laundering reporting duties.
