National Commission for the Promotion of Equality (NCPE) Comments Form
This form is completed by the person who has identified alleged discrimination at the place of work, (and/or) by banks and financial institutions, (and/or) in education and vocational training or guidance (and/or) in the provision of goods and services as well as in freedom of movement for workers in the EU, as per NCPE's remit. This form is for information purposes only. If the person wishes NCPE to start an investigation, s/he can fill in NCPE's Complaints Form.
National Commission for the Promotion of Equality (NCPE) Complaints Form
This form is to be filled in by complainant who alleges that s/he has been a victim of discrimination on the basis of sex or family responsibilities, sexual orientation, sex characteristics and gender expression, age, religion or belief, racial or ethnic origin, and gender identity in employment, banks and financial institutions as well as education and discrimination on the basis of racial or ethnic origin and gender in the provision of goods and services and their supply, and in the freedom of movement for workers in the EU.
Consumer Affairs Complaints Form
When you as a consumer do not manage to amicably resolve a dispute with a trader and hence require assistance from the Office for Consumer Affairs, you will need to lodge a complaint by filling in the complaint form. In this form you are required to provide your personal details, the details of the trader you have a dispute with, information on the type of product or service purchased, and also a brief description of the problems encountered. Together with the complaint form you are also required to attach copies of the documents related to the problematic purchase.

Complaint to Public Authority regarding a Freedom of Information Request
If an applicant is not completely satisfied with the way his/her Freedom of Information request was handled, s/he can complain to the public authority through its internal complaints procedure, and a reply to the complaint is provided within ten (10) working days. One should submit his/her complaint to the public authority not later than thirty (30) working days from the last communication of the public authority or the act or omission giving rise to the complaint, whichever is the later date.
