Submission of an application for recognition of the status of a participant in an anti-communist resistance pursuant to Act No 219/2006 Coll. The granting of the status of a participant in an anti-communist resistance is decided by the Institute of National Memory on the basis of a written application. The application contains the name and surname of the applicant, date and place of birth, permanent address, information on citizenship, data confirming fulfillment of the conditions for admission to the status of anti-communist resistance and handwritten signature. |
