Registering for an eID Account
The eID, or electronic identity, is a trusted authentication mechanism for citizens and businesses to identify themselves to electronically access services from across government. You must have an eID to use eGovernment Services.
Application for New Maltese Passport/Maltese Passport Renewal
This form is used by every Maltese citizen when applying for a Maltese passport for the first time, renewing or for the issuing of a new passport following a report of loss, theft or damage.
Declaration of Oath (Passports - Form J)
This form is used when an individual applying for a passport does not know the parents' details and in spite of exhaustive searches carried out at the Public Registry and Parish Offices, the relative certificate could not be traced.
Image Capture Application (Form B)
This form must be presented together with Form A whenever the live-biometric image of the applicant cannot be taken.
Parental Consent for Applicants under 18 years of Age (Passports – Form I)
This consent form is used whenever applicants have not yet reached the age of 18 and need to be completed when one of the parents lives abroad or is not in Malta. The form must be signed by the parent who is abroad and in the presence of a recommender. The recommender needs to have known the parent for at least 2 years. Persons qualifying as recommenders include Commonwealth Citizens, Medical Practitioners, Lawyers and Architects. This form is an extension of Section 7 of Form A, which should be delivered in person to any Maltese Representation overseas, including Embassies and Consulates.
