Products of Quality National Scheme (Animal Based)	This form is used to register and start the process for any interested farmers and co-operatives to join the Products of Quality National Scheme. The Scheme is intended to improve and certify the quality of the Agricultural
Beekeeper Registration and/or Honey Producers	A registration form for those persons who are interested in beekeeping or who keep honey bee colonies.
Official Notification of Births of Calves and/or Deaths of Bovines	This form is necessary so that bovine producers that have births on their farms will be in a position to notify the Department as it is required by law.
Rabbit Breeders Registration	A registration form to be used by persons who are interested in breeding rabbits.
Report of Death of Equines (Horses)	Through this application form, an applicant can report the death of equines (horses). The horse will then be removed from the National Livestock Database.
Transfer of Ownership of Equines (Horses)	Through this application form, an applicant can request to transfer equines (horses from present to new owner).
Animal Health Certificate	Form to be used by Veterinarians when certifying an animal’s general health and suitability for use on the road. Form is supplied by TM to VETs in booklets consisting of 3 colored copies per certificate.
Veterinary Surgeon Permanent Registration and/or Warrant	To practice as a veterinary surgeon in Malta, one must register and get a warrant.
Licence to keep Swine as a Pet	This application form has to be filled in by anyone who would like to keep swine as pet.
Pets Arrival Notification	A pre-notification of arrival of a pet into Malta with the arrival details. With this form there is control of all the pets that enter Malta, and if not fulfilled the pets' entry to Malta will be denied. This online pre-notifcation is used for both commercial and non-commercial movement.
Registration as a Keeper of New Dangerous Animals	Through this form, applicants may ask for the permission to keep a dangerous animal or any offspring born from a dangerous animal already registered in Malta, as per SL 439.19.
Report of Death of Pets	Through this application form, an applicant can report the death of any pets. The pet will then be removed from the National Livestock Database.
Report of Missing Pets	Through this application form, an applicant can report a missing dog or cat.
Transfer of Pets	Through this application form, an applicant can request to transfer a pet from one individual to another. This is mainly used for dogs and cats from present to new owner.
Integrated Pollution Prevention and Control (IPPC) Form A (Farms)	Farm rearing poultry or pigs falling within scope of the thresholds stipulated in Schedule 1 of the Industrial Emissions (IPPC) Regulations (S.L. 549.77) require an Integrated Pollution Prevention and Control (IPPC) permit prior to operation.
Slaughter Booking Form	Enables farm animal owners to book their animals for slaughter.
